when i got a new phone, i installed the same game app and logged in with my Facebook account (one of their log in options) which was previously tied to my progress. worked like a dream! :) so i'd say just try to log in with same game account you used on old phone.
You may want to download your ministry id if you put time into it. I don't think it is saved except on your phone. You can upload it to your new phone now I believe.

Yep. You should be able to just login with the same method you used originally and should be good to go. :)
