There's 2 parts to this issue.
One, like you said, the absolute lack in testing and quality control.
Second, the results of lack of said testing and control.
Yes, with sufficient testing this could be prevented. But it didn't. It happened.
Now since it went live, they didn't promptly acted whenever this "feature" was put under there attention. They waited weeks to clarify and even longer to fix it. The only response we basically got was "oh well, here's a free pack, move on". And that's why everyone is upset.
Not that they goofed. Everyone knows that it will happen. It's that they don't care.
Yes, in the grand scheme of things, the RSB aren't important. But the lack of caring about their customers is what ticks everyone off.
No testing and quality check is one thing. Giving the finger to everyone as a response... That's the issue.
to me this train of "incidents" feels like (some) unhappy employee messing up the system/server. 🤔
What I'd like to know at this point about the trace charm is why fully half the times I cast it on a red beam trace it gives me Fluffys. I'm so sick of Fluffy and the 4 casts it takes to beat it. Super time consuming. Make some other traces appear!
So, if not fragments, what is that +1 supposed to mean??? Come on ...
If you really will not give what's in the description, I suggest you increase significantly the family XP per trace. I'd say 20 FXP per proficiency level sounds fair.

I’m not even Consistently getting one additional fragment. There is frequently a glitch and it keeps getting lost!
It initially reports giving me one additional fragment, two total, but at the end shows it actually only gave one, and when I check in registry it in fact only gave one.
Here is an example. At first it increases from 5 to 7, but then at the end only increases to 6!
@EliasHolmes Is this trace from a Dark Detector? I routinely get only one fragment from one of the two traces from a Dark Detector.
We should be able to do a refresh of our SOS skills and start over, reading the other comments I see others agree with me I wouldn't have spent RSB on these nodes if I had known this. I also would have gone other directions with my DADA books. If we could clear our skill trees and start over we would be happier.
Anyone suggesting them to clear out our skill trees, so we can start over needs to think back to the "fragment fixes". What happens if they try that and many players suddenly lose all their guides and books? All we will get for the losses would be another nearly worthless gift! Stop suggesting refunds, when the result can be another major catastrophe!
@BenTiger exactly the point I tried to make. A reset would almost certainly cause more problems than it solves.
The way you are handling this is really frustrating. We would appreciate it if you would be more honest and transparent about the "errors" regarding this charm/game mechanic. The errors here are all on WB/Niantic/Game Developers for not more thoroughly walking through the new features themselves before releasing them to the players. The players didn't misinterpret anything. And I don't feel like you are properly acknowledging those two facts.
We get a "free" Trace Proficiency Node at Lvl 25. It says "This knowledge enhances the power of your Trace Charm, increasing the number of fragments you return with each cast of the spell." And below it shows "+1". If this is the ONLY Trace Proficiency upgrade a player has, the effect after attaining it is that Traces/Foundables returned from using the Trace Charm now return TWO fragments, instead of ONE. So, when reading ALL the other Trace Proficiency Lesson Descriptions, and ALL of them ALSO state "Increases the number of Fragments you recover whenever you use your Trace Charm" or "increasing the number of Fragments you can recover from those Traces" or "increasing the number of Fragments you recover from Traces affected by your Trace Charm" and they ALL say "+1" it is PERFECTLY reasonable to conclude that getting these lessons will increase the fragments obtained per return. And NEVER ONCE in ANY of those descriptions does it communicate an increase to Family or Wizarding XP, or show how MUCH the increase will be. The only way we knew this was happening was just by noticing it on our rewards screens. And although our fragment count never increased by more than 2 per trace returned, with each Proficiency Lesson completed, the Family and Wizarding XP DID continue to increase.
The wording of this is communicated too clearly, and repeated too many times for it to just be an "oooooops!" Therefore, it is disingenuous to try and say that the ORIGINAL intent was to ONLY ever get a maximum of 2 fragments per successful return from Trace Charms. You also can't reasonably convince me that all of the other nodes were originally JUST supposed to be the Family and Wizarding XP increases, since this wasn't mentioned ANYWHERE in the descriptions.
Maybe the lessons were planned before the fragment count update?? Maybe they were *supposed* to change the wording to all of the Proficiency Lessons other than Lost and Foundables (the free lvl 25 lesson - which does have a slightly different icon than the rest of the Proficiency or Efficacy lessons) to reflect the increase in Family and Wizarding XP, OR take out all of the other proficiency lessons altogether, but someone dropped the ball before releasing it? I'm not sure where the breakdown happened; but either way, it wasn't on the players side; we did not misinterpret anything.
What I believe is really going on here is that when players got a hold of the SOS training, and started using the Trace Charm, the game developers FINALLY REALIZED how ridiculously overpowered it would be to ultimately give you 9 fragments PER successful return from traces revealed by the Trace Charm. (There are 8 Proficiency Lessons, so, including the original you would always get, that totals 9.) After all, you only need 10 from an Emergency Threat to place on a Gold frame. And if you are getting a total of 10 traces (the original, the duplicate, plus the 8 NEW spawns) that EACH give you 9 fragments upon return... you'll get to place an image at LEAST every other one, meaning placing 4-5 times PER Trace Charm used. Which means ranking 24-30 times, giving you a huge chance at getting more Trace Charms, and making an "infinite loop" all the more likely. So, I GET it, the Proficiency needs to be nerfed. One of the reasons my husband, son, and I went so hard using dark detectors and trace charms from the get-go was because we realized this almost immediately; figured it would get nerfed, and wanted to take advantage while we could.
But, be honest about it, guys. Just SAY "So... I know the lessons SAY you should ultimately get 9 fragments, and that it isn't currently working that way, but... we're not going to fix it. We realize now that it would be too overpowered, and are going to leave it at a max of 2. But, those lessons will still give the extra Family and Wizarding XP bonuses!" THEN, you REFUND the resources spent on those Lessons (even if it's JUST those specific nodes) to the players who have purchased them. Then, you CHANGE the wording to ALL the other Proficiency Lessons to ACTUALLY reflect the Family and Wizarding XP bonus, and change the number to "+0.1" or "+10%" (or whatever the actual math is) so that no other players make decisions based on false information. However, I also think that the costs of these skills should be reconsidered, because the tiny little increase isn't worth the costs of the original lessons. And honestly, I wouldn't be sad if you took away the extra Family and Wizarding XP, and did away with all the Proficiency Lessons completely. At least, so long as you actually refund the books I spent on them first!
I have all but two Trace Proficiency Lessons: Repository Knowledge and Fragmented Memory, because they are both blocked by high DADA cost skills. Which means I have spent 2,703 Field Guides, 883 Ministry Manuals, and 43 Restricted Section Books on Proficiency Lessons. So, luckily, I haven't spent DADA Books on the ACTUAL lessons. However, I DID just spend 14 DADA Books getting "A Horse With No Name" (overall departure rate) SPECIFICALLY to get to "Piece of the Puzzle". So, your "Complimentary Gift" of 75 Field Guides, 20 Ministry Manuals, and 2 DADA Books doesn't actually refund me the resources spent due to your errors. And while I may have eventually gotten the overall departure rate, I definitely would have prioritized differently if I knew that a +0.1 Family and Wizarding XP bonus was the only Trace Charm bonus waiting on the other side. Originally, players get 20 Family XP per Emergency Threat. With 6/8 Proficiency Lessons completed, I get 33 Family XP per Emergency Threat. It's not useless/pointless, but... it's definitely NOT equivalent to getting the additional fragments.
Please reconsider how you are handling all of this. Try to find a way to fix/alter it that ACTUALLY makes players feel like you are making up for your mistakes. Numerous suggestions have been made, many of which would be better than what you are currently offering.

The one to the right that says maximum spell casts 0, that is showing how many spell casts you currently have, we all start with 2 right so 0 is wrong. You should either show maximum spell casts and it be the total they can hold, or change the description to current spell casts held as that is what it is currently showing. My changes from 0 to 5 depending on how much I have at the time I look at it.

Another disappointment. Everyone wants to help you guys avoid these issues so much, but when all suggestions are just outright ignored, it doesn't make anyone feel better.
One of the strongest parts about Ingress was the community. They were listened to, and some great ideas came out of it. Ever since the money from PoGo started coming in though, community was listened to less and less. You guys have so much potential, and we know things don't go right sometimes... But to see you constantly tripping over these issues, then completely fumbling these "compensations" makes you appear tone-deaf and uncaring.
Being a software programmer and myself back in the day, I really do get that even doing the most thorough bug checks, there can still be glitches that fall through the cracks. Thankfully I have not wasted green books yet on the increased fragment counts. I just want to be another voice advocating for those who have. As everyone knows, the green books are highly valuable and rare. It's not about inconvenience this time, it's about simply having good and ethical businesses practices. If we bought something with muggle money and it turned out the product wasn't as advertised or defective, even unintentionally, a good business would refund the money no questions asked. I know how databases work. There is no reason why you can't do a search of all the players who have spent green books on those nodes and credit their account. We love this game so much. Please don't put the burden on those of us who value ethics over fun, the hard decision of having to uninstall the game and stop playing. Please do the right thing. Thank you.

Speaking about clarifications, there are two ---very expensive--- nodes in the SOS trees that requires clarifications. I have read some theories about them but nothing official. It should be a good idea to clarify them before we can actually buy them, to avoid further "drama". Those nodes are :
Regarding "Preferred client", is it a gift containing gold that we can send to friends (it is written gift but such gifts currently don't exist in the game and we usually don't receive gifts in treasure trunks) or is it gold that we receive as a gift (but the icon and the term gold gift are confusing).
Regarding "Sage advice", there is another node with exactly the same phrase without the word "Rare". I would guess that the other increases the chance to receive "extravagant runestone gifts" (that contains spellbooks). But what are Rare spellbooks ? Some people hope that it would be DADA books but they are probably dreaming. This needs clarification...

@Ashkaa The Extravagant Spell Energy gifts are awarded from the "Rare XP" and "Rare Energy" gifts. The "Rare Spellbook" is just another regular Spell Book. It is just another case of a poorly worded skill. They obviously did not put forth any effort to check for issues with skill descriptions.
It’s pretty clear this needs to be addressed. The “Dangerous Attraction” bait and switch has demoralized me from being a $200/month player to $0 and barely having the interest to complete daily tasks. Without some acknowledgement and remediation, this Lvl 56 maxed A/M is going to find another outlet.
Okay, now you just did something to make the Trace Charm practically worthless. I just cast it on an orange beam and instead of giving me other orange beams it gave me on red beam (a worthless Fluffy) and 3 low level traces. I've been supportive but now you've gotten rid of EVERYTHING that was good about the Trace Charm.

Yeah I'm done with the trace charm too. I casted it on Orange beam da-trace, it gave me Orange hh-traces. While I was trying to catch those, it had changed the da-trace to friggin normal basic woww quibbler-trace. So why would I want to boost efficacy or even keep playing, when nothing works like it is supposed to. 4876 fragments catched and no mystery objects for the assignment. Trace charm doesn't work. Stupid disco effect and foe already engaged and everything, restarting all the time. I was so excited when I finally got a phone this game Works at, since i've been hc Potter-fan for years. You really have made me angry and truly dissapointed. I have loads of paramount portkeys, but I ain't using my money that I have earned with hard work to support this travesty of a game. Yeah, I'll still keep playing, but not paying for anything before things are fixed and you have apologized from those who have played from the start and put significantly more money in this game, than I who has played for little over a month.
I just posted a screenshot of the same thing happening to me and it went to moderation. Not sure why. The game is messed up though.

Now the trace charm is completely nerfed. Cast it twice today on red beam traces and it spawned one red beam and six or seven common random traces. Garbage. So now proficiency AND efficacy are worthless.
I find it amazing that yoiu bozos rush to "fix" any "problem" that benefits the players, but any of your problem that cheat and deliberately deceive the players are blatantly ignored. I've spent a lot of REAL money buying resources for this infuriating game, but I'm done. You will never get another red cent out of me. Your group has absolutely no integrity whatsoever.
